- Complete the Upgrade Request Form (hard copy, or available on Website under FORMS)
- Complete the Month-to-Month Request Form (hard copy, or available on Website under FORMS)
- Clients must complete Stacking Request Form prior to the end of their original (hard copy at front desk, or email, email@example.com)
Clients who are unable to maintain the frequency of their Pricing Agreement due to extenuating circumstances can extend the expiration of their remaining sessions up to 60 days. Payments will continue to be made, and sessions will remain available for use. Any sessions that are not used within the Pricing Agreement period can be accessed for up to 60 days following the end of the Pricing Agreement. Upon completion of the remaining sessions, the client will have to purchase a new Pricing Agreement and will be subject to new/current pricing.
